Please note: Your plan may not cover outpatient laboratory services provided by a hospital or the hospital's subsidiaries. Your plan only covers lab services at these locations when provided as pre-op, emergency room or urgent care are covered by your plan. Please pay close attention to the address listed to ensure the provider is in network at that location. Contracts can be location specific so it is important to verify the location where you will be receiving services. Please contact your Health Plan's claims administrator for additional information.

Members of our provider panels are required to meet strict credentialing standards, and we monitor the overall effectiveness of our providers regularly to ensure that they continue to meet our standards.
